He's not been taught how to take an impression in a foam box.
But he does say "This is a demonstration of pepole who use foam boxes..." he did not say this is how it is done.
Must have a real axe to grind with Dr Scoll to go to that effort

If a Podiatrist sent a patient to me with all the symptoms you are discribing i would try the orthotics with the pmp and u. also ensure that there is sufficient padding and that her footwear has a rounded toe box.

Subluxed patella and orthotic treatment ?

I have found orthotic intervention in patellar tracking problems less effective than isometric exercises. The Vastus Medialis Obliquus (VMO) acts to pull the patella medially during the last 30 degrees of extension, while the other quad muscles exert a lateral pull on the patella. The VMO in a large segment of the population is less developed than the middle and out quads and I have found this to be the case often in patellar tracking complaints.
